Separate sealed BIDS for the construction of WATER SYSTEM IMPROVEMENTS – PHASE III will be received by the Mossy Head Water Works, Inc. until 2:00 p.m., local time, JANUARY 22, 2009 at the Water Office at 1485 County Highway 1087, DeFuniak Springs, FL 32433. BIDS received after this time will not be accepted. BIDS will be publicly opened and read aloud.

Separate BIDS for EACH CONTRACT will be received as follows:

CONTRACT I – WATER LINES: Work on the Project consists, in general, of construction of approximately 7,600 LF of 12” PVC water main; 46,000 LF of 10” PVC water main; 30,100 LF of 8” PVC water main; 54,500 LF of 6” PVC water main; 213 gate valves; 112 fire hydrant assemblies; 5,380 LF of directional bore; service reconnections; and all appurtenances necessary to complete the project. All work shall be completed within 300 consecutive calendar days.

CONTRACT II – TANK NO. 3: Work on the Project consists of constructing a 200,000 gallons elevated water storage tank, site work, and all appurtenances necessary to complete the project. All work shall be completed within 210 consecutive calendar days.

The Contract Documents may be examined at the following locations: (1) Mossy Head Water Works, Inc., 1485 County Highway 1087, DeFuniak Springs, FL 32433, (850) 892-6154, and (2) Peters Municipal Associates, Inc., 300 North Foster Street (P.O. The Walton County Zoning Board of Adjustments will hold their regular public hearing on January 22, 2009 at 6:00 P.M. at the South Walton Courthouse Annex in Santa Rosa Beach, Florida 32459. The following items are scheduled for review and action:

1. BUDDIG SUBDIVISION ADMINISTRATIVE APPEAL – Project number 08-004-00003. Scott Work on behalf of Buddig Subdivision is appealing an interpretation by Pat Blackshear, Director of Walton County Planning and Development Services Division, dated February 8, 2008. The interpretation stated that (1) Director Blackshear was designating the project a major development instead of a minor development pursuant to Section 11.01.02.C.6(b) of the Walton County Land Development Code, and (2) Director Blackshear was requiring the applicant to submit a compatibility analysis pursuant to Section 11.02.03.D.10 of the Walton County Land Development Code. The Zoning Board of Adjustments will make a final determination to grant a variance, deny a variance, uphold the director’s decision, overturn the director’s decision, or continue to a later date. Approximately 660 feet of concrete ditch will be removed and replaced as part of this project. The proposed roadway will consist of 3.0 inches of SP 12.5 asphalt. The existing sand/clay base will be mixed in place and shall have a minimum LBR of 70. All disturbed areas along the proposed improvements shall be sodded.

6. WALTON COUNTY APARTMENTS PUD – Project number 08-001-00055. This is a major development order application submitted by Connelly & Wicker, Inc., requesting conceptual approval of 261 multifamily rental units within 9 buildings and 7,700 square feet for office/bank use on 46.27 +/- acres with a future land use of small neighborhood. The site is on the east side of U.S. 331, approximately 1,600 feet south of Chat Holly Road (parcel numbers 29-2S-19-24000-004-0000 and 29-2S-19-24000-008-0000).

Installation of Baseball Field Lighting
Walton Senior High School

The Walton County School Board is seeking Request for Proposals (RFP) to install baseball field lighting at Walton Senior High School in DeFuniak Springs, Florida. The lighting and tower structures are being provided by direct purchase through Musco Lighting. Contractors will be required to install 6 – 70’ Musco Lighting Structure System (including light fixtures) and the demolision of 8 – existing wood poles for storage on site. Contractor is also required to provide and install 2 – new 200 AMP, 1 Phase, NEMA 3R Fused Disconnect Switches on existing 400 AMP Service to control new lighting. All wiring will be direct burial and some existing wiring may be reused if applicable.

Contractors shall submit their RFP in a sealed envelope marked “RFP Walton High School Baseball Field Lighting” to the Office of the Superintendent of Schools, Walton County School District, 145 Park Street, Suite 2, DeFuniak Springs, Florida 32435. RFP’s shall be received no later than 2:00 pm, Thursday, January 29, 2009.
The Walton County School Board shall have the right to reject any or all RFP’s and shall have the right to award the RFP in the manner the Board determines to be in its best interest. All respondents are notified that failure to file a written Notice of Protest with the Superintendent of Schools within seventy-two (72) hours after the receipt of notice of the decision or intended decision of awarding the RFP shall constitute a waiver of any further right to protest the RFP award.

The administration of the Estate of Gerald W. Johnson, deceased, whose date of death was October 1, 2008, is pending in the Circuit Court for Walton County, Florida, File No. 09CP000001; the address of which is Walton County Courthouse, 571 U.S. Hwy. 90 East, DeFuniak Springs, Florida 32433. The name and address of the personal representative and the personal representative’s attorney are set forth below.

All creditors of the decedent and other persons, who have claims or demands against the decedent’s estate, including unmatured, contingent, or unliquidated claims, and who have been served a copy of this notice, must file their claims with this Court WITHIN THE LATER OF THREE (3) MONTHS AFTER THE DATE OF THE FIRST PUBLICATION OF THIS NOTICE OR THIRTY (30) DAYS AFTER THE DATE OF SERVICE OF A COPY OF THIS NOTICE ON THEM.

All other creditors of the decedent and other persons who have claims or demands against the decedent’s estate, including unmatured, contingent, or unliquidated claims, must file their claims  with this Court WITHIN THREE (3) MONTHS AFTER THE DATE OF THE FIRST PUBLICATION OF THIS NOTICE.

REQUEST FOR
ENGINEERING PROPOSALS
CITY OF DEFUNIAK SPRINGS
PHASE III – WATER SYSTEM IMPROVEMENTS

The City of DeFuniak Springs, Florida is soliciting engineering proposals from qualified firms interested in providing professional engineering services in conjunction with the City’s proposed Phase III Water System Improvements Project funded by the USDA-RD Program. The estimated project budget is $5.8 Million which includes engineering, design, and construction. The project is the construction of water main extensions and/or upgrades, a 500,000 gallon elevated water storage tank, and 1,000 GPM well.

Professional services required shall be secured in accordance with Florida Administration Code Rule 9B-43, 24 and CFR 85.36 and by Section 287.055, Florida Statute (Consultants’ Competitive Negotiations Act). The deadline for sealed proposal submissions is 2:00 P.M., January 28, 2009 and can be delivered to City Hall at 71 US Hwy 90 W, DeFuniak Springs, FL 32433 or mailed to P.O. Box 685, DeFuniak Springs, Florida, 32435. Proposals shall be labeled “Professional Engineering Services: Water System Improvements Phase III.” Proposals received after the scheduled submittal deadline will be returned unopened. Please submit 1 original and 9 copies of RFP’s. No oral, telegraphic, electronic, facsimile, or telephonic proposals or modifications will be considered. The City seeks to engage a firm with an office within 100 miles of DeFuniak Springs who’s Engineers are licensed in the State of Florida and which has extensive experience in all facets of the services required.
