This project consists of installing 16 locations of overhead lights along the mooring pier at Four Mile Creek Park on Lafayette Creek. The project scope includes the following items:

* Provide and install 8’ x 3” galvanized posts for light fixtures at location shown on plans.
* Mount lights on light posts per all applicable codes (lights to be provided by the City)
* Provide and install conduit and wiring from lights to nearest transformer box located at North side of restroom per all applicable electrical codes.
* Provide and install photo cell electrical panel (chosen by bidder) to control lights.

The City of Freeport will hold a MANDATORY pre-bid meeting on-site to review site plan and answer any questions on September 4, 2009 at 4:00 pm (CST).

Petitioner alleged in its Petition that you are a possible biological father of the Child and that you, having actual knowledge of the birth or of the impending birth of the Child, and having statutory presumptive notice that the biological mother may be pregnant and actual knowledge that the biological mother was pregnant and that you may be the biological father of the Child, have: (1) failed to prove you are the father of the Child; (2) failed to exercise parental rights or duties over the Child, including but not limited to having failed to contribute to the support, either monetarily or non-monetarily, of the Child’s biological mother or the Child to the extent of your financial ability since the date of conception of the Child; (3) failed to make sufficient attempts to discover if you had fathered the Child prior to the receipt of notice; (4) failed to make sufficient attempts to exercise parental rights and duties toward the Child prior to the receipt of notice; (5) to the extent you are or may be entitled to custody of the Child, abandoned the Child; and (6) failed to establish or create a legally-existing parental relationship with the Child.

YOU ARE, THEREFORE, NOTIFIED THAT the Court will consider the Petition for  Termination of Parental Rights of the Biological Parents of Baby Boy Newman, a/k/a Michael Joseph Newman on the 11th day of September, 2009, at the hour of 1:30 o’clock P.M., before the Honorable Jesse S. QUASI-JUDICIAL ITEMS:

T-MOBILE WIRELESS TELECOMMUNICATIONS FACILITY – Project number 09-001-00017.  This is a major development application submitted by Thad Watson, as agent for Powertel/Memphis, Inc., d/b/a T-Mobile.  This application seeks conditional use approval for a 250’ self-support tower and related telecommunications equipment, within a fenced compound on .23 acres with a future land use of general agriculture.  The site is at 5389 C.R. 192 (parcel number 36-4N-20-29000-004-0000).

3.    PARADISE RETREAT PETITION FOR ABANDONMENT – Project number 09-008-00002.  This is a petition for abandonment submitted by Eddie R. Grant, as agent for the Paradise Retreat Homeowners Association.  The petition is to allow the homeowners association to privatize the streets within Paradise Retreat per Florida Statute 336.125, more specifically Sandy Cay Drive, Secret Harbor Drive, and Marigot Bay Circle.  The subject of the petition is located entirely in southwest Walton County, and includes all roads within the Paradise Retreat subdivision, from the intersection on the south end of Sandy Cay Drive and old U.S. 98 (now Scenic 98) and the intersection on the north end of Sandy Cay Drive and new U.S. 98 (Emerald Coast Parkway West).  The future land use is coastal center.  The site is on the south side of U.S. 98, just east of the Walton County and Okaloosa County line (parcel number 30-2S-21-42790-AAA-AAAA).

4.    THOMPSON WOODS F/K/A ASSISI VILLAS REPLAT – Project number 09-003-00001.  This is a plat application submitted by Assisi Development, LLC and Rosewood Development, LLC, requesting the re-plat of a previously approved development known as Assisi Villas.  The re-plat proposes to change the subdivision name from Assisi Villas to Thompson Woods, and to rename a roadway Woodshire Drive. The site is 1300 feet south of U.S. 98 on Thompson Road (parcel number 34-2S-20-33400-000-0020).

Separate sealed BIDS for the construction of STATE HWY. 20 SIDEWALKS FROM K. The City of Freeport is seeking professional consultant services for Construction Engineering Inspection for the construction of approximately 900 linear feet of sidewalk located along State Road 20 commencing at Kylea Laird Drive and proceeding east to the Auto Zone Auto Parts driveway. Consideration will be given to only those firms that are qualified pursuant to law and that have been prequalified by the Florida Department of Transportation (FDOT) in accordance with Rule Chapter 14-75, Florida Administrative Code to perform the indicated Type(s) of Work.

Letters of Response are limited to five pages for this project.
Letters of Response should at a minimum, include the following information:
a.    Project Name/DOT Financial Management Number:     426719-2-58-01
b.    Consultant’s name and address
c.    Proposed responsible office for consultant
d.    Contact person, phone number and Internet Email Address
e.    Statement regarding prequalification of consultant or sub-consultants in advertised type of work
f.    Proposed key personnel and their proposed roles (do not include resumes)
g.   Sub-consultant(s) that may be used for the project
h. Indication as to whether the prime firm and/or sub-consultants are disadvantaged business enterprises     (DBE)
i.    The outside of the envelope should be marked with “Sealed Proposal  Sidewalk Project SR 20.”

This project is funded through the American Recovery and Reinvestment Act of 2009(ARRA). Sections 120 and 1512 of the American Recovery and Reinvestment Act require States to fulfill employment reporting obligations for each ARRA funded project. The prime consultant on each ARRA project shall complete an employment report for each month from the date of the Notice to Proceed until completion of the contract or September 2012, whichever occurs sooner. Consultants are required to provide the necessary information for their own workforce as well as the workforce of all sub-consultants that are active on their ARRA funded project(s) for the reporting month. A full description of the data elements to be reported will be provided to you at a later date as revisions to the contract.

The Freeport City Council reserves the right to reject any and all proposals deemed in the best interest of the County.
